Complete, typed or neatly filled (In Capital Letters) and signed applications, along with the following supporting documents, must reach the coordinators at the address mentioned below, by 12th September 2006

The completed application should include:

1. Curriculum Vitae of the Applicant
2. Two Passport size Photographs of the applicant
3. One copy to be affixed in the application form / One copy to be sent in a separate cover
4. Photocopy of Degree Certificate
5. Photocopy of Medical Council Registration Certificate
6. Course fee of Rs.1,000/= to be paid as A/C Payee Draft in favour of ‘IASSTD & AIDS” payable at Chennai, on receipt of selection intimation by the participant and send to the Course Coordinators on or before 27th September 2006

Last Date for acceptance and payment of course fees by the participant: 27th September 2006
If DD and acceptance letter are not received by 27th September by Course Coordinators, the candidate forfeits his/her selection
Dates of the STIs Certificate Course : 11th October to 19th October 2006
